Step 1: Referrals
Methods - (self, organisation, educational body, statutory body). Referrals are made using Family Services external referral form .
Step 2: Assessment
Clients are invited for an assessment within 10 working days of the referral. Method - Assess client needs, identify concerns, and gather information of family history, present circumstances, and future progress. Talk through where the client would like to see themselves in 3 months and their expectations of the support they would receive from Family Services UK. Type of intervention - Based on the requirements of the referral agency or discuss with client the best option and come to an agreement.
Step 3: Review
Discuss assessment at clinical supervision within 24 hours. Review assessment at managers meeting within 48 hours.
Step 4: Allocation
Assessment passed on to appropriate specialist i.e. clinician/counsellor/mentor/family worker. Intervention starts within 3 weeks of client being allocated to a specialist. Specialist to meet with client/family to agree contract and length of intervention.
Step 5: Supervision
Weekly clinical management meetings to discuss client/family case progress and monthly supervision to explore difficult areas and support approaches.
Step 6: Monitoring
Monitoring form carried out every 4 weeks.
Step 7: Reassessment
Four weeks to ending contract discuss care plan with client. If further intervention proves necessary, a two week reflective period is given to client and then an appointment for a reassessment. See Step 2.
Step 8: Signposting
If a client does not need further intervention or help but could benefit from other services, i.e training, volunteering, legal help or other assistance, then sign post client and discharge them.
